Instructions on how to fill the Application Form

• 1. An applicant need to download and complete ONLY one set of Application Form.
• 2. Application Form should be in BLOCK LETTERS with all the relevant Information as required.
• 3. Passport size photographs should be attached at the right-top-corner of the form.
• 4. An applicant should attach all relevant certificates/result slips and other qualifications.
• 5. The College shall not be responsible for any mistakes on the part of any applicant.
• 6. Applicant’s Names must correspond to those used for all examinations taken.
• 7. Applicant’s must scan and submit the completed form to the college via email info@rch.edu.gh
• 8. Applicant’s must pay a processing fee of 100 Ghana Cedis to the MTN Mobile Money number 0548321881
• 9. Finally Applicant’s must call the college on 0554168740 for verification
